Obverse: Inscribed "SIGISMVNDVS.III.D:G:POLONIAE.ET.SVECIAE.REX". Reverse: Inscribed "MAGNVS.DVX.LITVAN:RVSS:[ ]:PRVSSMAS:SAM:LIVON:ZC': [two rosettes]".

Signed

Obverse: Below to the left signed "S.A." and dated "1621". Reverse: Signed on each side of arms. "-VE" and dated in the center "16-21". Signed below "S-A" and dated again "16-21". Signed again beneath the shield "S-A".
